Efficacy: In someone genuinely deficient, correcting vitamin D levels has real, replicated benefit for bone density and fracture risk. In someone who isn't deficient, the evidence is far weaker: the VITAL trial — nearly 26,000 participants — found no significant reduction in cancer or cardiovascular disease from supplementation, directly contradicting the broadest marketing claims built around this vitamin over the past two decades.

Safety: Very safe at standard doses. Sustained high-dose supplementation can raise blood calcium to levels that affect the heart and kidneys — a genuine risk that has grown more relevant as high-dose vitamin D products have become more popular.

Efficacy: B12 deficiency is real and reasonably common — in older adults, people on certain medications (like metformin), and those following a vegan diet. What the gap actually shows

Six of the twenty-five best-selling supplement categories in America — multivitamins, vitamin C at typical doses, glucosamine/chondroitin, biotin for cosmetic claims, echinacea, and saw palmetto — carry "Weak" strength of evidence for the primary reason people buy them. Several of these are also among the highest-revenue categories in the entire market. Commercial success and clinical support are simply not the same axis, and nothing about a product's shelf placement, price, or popularity moves it from one axis to the other.

The best-evidenced entries on this list — creatine, protein powder, vitamin D and B12 for deficiency correction — share a common feature worth noticing: each makes a narrow, specific, testable claim rather than a broad promise. The weakest-evidenced entries tend to make the broadest claims — general "immune support," general "energy," general "anti-aging."
